New Patient

Thank you for your interest in working with JEN Lifestyle Medicine. Please review our website content for additional information. Please do not hesitate to call us or email us with any questions.

Please note that JEN Lifestyle Medicine is a consultation and holistic aesthetics practice. We are NOT primary care. You are recommended to have or retain a primary care physician. We do not see Medicare or Medicaid patients at this time due to federal and state law regulations as they apply to our clinicians.

At this time, JEN Lifestyle Medicine only accepts patients aged 18 and over for its integrative lifestyle medicine consultation.

JEN Lifestyle Medicine visits are by appointment only. We offer a FREE 15-minute consultation in person for eligible patients who are interested in the integrative lifestyle medicine consultation or cosmetic acupuncture. This consultation will give you an opportunity to discuss your goals with Dr. Lin and for us to mutually determine if you are a good fit for JEN Lifestyle Medicine.

Payment for all services is required at the time of care. We accept debit and credit cards.

Insurance Policy

JEN Lifestyle Medicine operates as a self-pay medical practice. This means that we do NOT accept health insurance and we are NOT a participating provider with Medicare or Medicaid and we will NOT file insurance paperwork on your behalf. We can provide you with detailed statement (superbill) that contains diagnosis and procedure codes submittable to insurance plans with “out-of-network” benefits. However, we cannot guarantee that services provided by JEN Lifestyle Medicine will be reimbursed.

You may be able to use medical spending accounts, health savings accounts, and flexible spending accounts to pay for treatment. Should you have any questions, please contact your human resources department or health insurance company directly.

When you schedule your first appointment, we request a credit card on file to hold the appointment for you. However, no charges will be applied prior to your appointment unless you cancel or reschedule your appointment without proper notice.

Cancelation Policy

We are committed to providing all of our patients with exceptional care. A substantial amount of time is used to prepare for each of your appointments. When a patient cancels without giving enough notice, this prevents another patient from been seen.

JEN Lifestyle Medicine has a 72-hour cancellation policy. Cancellation within 72 hours of appointment, or no-shows for appointments will incur a cancellation fee equal to the amount of the scheduled appointment. There is no fee to reschedule or cancel an appointment if requested outside of the 72-hour minimum cancellation policy.

Please Note: Holidays and weekends do not constitute business days. Although you may receive reminder phone calls, emails, and text from our office about appointments, these reminders are a courtesy only. It is your responsibility to remember your appointment date and time.

If you arrive more than 15 minutes late for your appointment, you may be rescheduled to ensure timely treatment of other patients and a missed appointment fee will be charged.

There is no charge for inclement weather cancellations or for severe illness cancellations. In the event of an inclement weather, our office will be closed. You will be contacted to reschedule your appointment.
